About The Course:

The patient is brought into emergency with a compromised airway. When intubation and supraglottic airway methods fail a case of difficult airway is identified and cricothyroidotomy performed to secure oxygenation and ventilation. 

This multiplayer open scenario VR session is designed to enable learners to identify and manage a case of difficult airway and perform a cricothyroidotomy. 

Learning Objectives:
  • Recognize and declare difficulty in airway management promptly 
  • Utilize a simplified, single algorithm for managing difficult airway intubation 
  • Aim to minimize trauma from airway interventions by limiting the number of attempts and avoiding blind techniques 
  • Advocate for the practice and familiarity with scalpel cricothyroidotomy as the preferred rescue technique

Difficult Airway Management

As part of this open scenario, the learner will begin by intubating the patient. To do this the laryngoscope is inserted but after two failed attempts the learner will try to secure airway patency using supraglottic airway. When this fails too, the learner will proceed to perform a cricothyroidotomy. This involves placing a tube through an incision in the cricothyroid membrane, which will help secure an airway for ventilation and oxygenation.
This scenario is set in a realistic environment and all necessary affordances are made available to assist learners in performing their tasks with efficiency and effectiveness.

Debriefing

End-of-task debriefing to assess one’s performance, evaluate actions, and get the most out of the training. Examples of topics being touched upon in the debriefing include the following: 

  • Thorough patient assessment 
  • Completion of necessary steps 
  • Correct medication administration to patient 
  • Sequential completion of task 
  • Execution of time-sensitive tasks
