About The Course:

This tool for assessing a Warfarin Patient is designed to help learners practice their patient assessment skills.

Employing end-to-end AI and technologies like large language model (LLM) and deep learning, these virtual ‘clients’ possess the ability to respond like real humans. Learners can conduct a completely organic conversation with the client, asking them about their symptoms and problems faced. Based on the client’s responses, a line of diagnosis can be established, and treatment plan formulated.

The MedVR Ed XR Platform provides trainers and instructors with the facility to review conversation logs and analyze learner performance based on customized rubrics. Based on the performance review, trainers will be able to provide precise and effective feedback.

Learning Objectives:
  • Educating the patient on warfarin’s role in preventing blood clots after heart valve replacement
  • Identifying the appropriate INR target range and testing frequency
  • Discussing important drug-drug and food-drug interactions while on warfarin
  • Identifying signs of abnormal bleeding and clotting and counselling patient on management
  • Educating the patient on what to do for missed doses
  • Evaluating the use of the teach-back method

Case History

Assessing a Warfarin Patient

Esmeralda Santiago is a 58-year-old female and she is at the Outpatient Anticoagulation Clinic for routine warfarin (Coumadin) management following a heart procedure two months ago. She reports good adherence, with no missed doses, and her INR today is 2.1. She takes over-the-counter supplements including ginger tea, turmeric, and occasional ibuprofen, and has recently adopted a diet rich in leafy greens via green smoothies. She is unsure of her warfarin dose but takes “peach” tablets and has 25 left from a refill 17 days ago.
Your task is to assess Esmeralda’s adherence, identify potential interactions or factors affecting INR stability, and address her concerns about long-term warfarin use.
Use the information gathered to guide clinical decisions and provide the patient appropriate education and support.
