About The Course:

Jack Scott, a 35-year-old male, has arrived in the emergency department following a motor vehicle accident. He is positioned on a stretcher with a cervical collar and oxygen mask in place. To rapidly assess for internal injuries and guide further management, an E-FAST (Extended Focused Assessment with Sonography for Trauma) examination is required.

This VR simulation for E-FAST for trauma is designed to help practitioners practice in obtaining all five necessary views to complete a trauma “E-FAST” ultrasound scan. With both training and assessment modes, learners can perform the procedure and assess themselves on their performance.

Learning Objectives:
  • Obtaining the Right Upper Quadrant (RUQ) view to assess Morison’s pouch, liver tip, and paracolic gutter
  • Obtaining the Left Upper Quadrant (LUQ) view to assess the splenorenal recess, spleen tip, and paracolic gutter
  • Performing transverse suprapubic view to evaluate the perivascular space and Pouch of Douglas
  • Performing sagittal suprapubic view to further assess the bladder and adjacent pelvic spaces
  • Acquiring subxiphoid and parasternal long-axis cardiac views to identify pericardial effusion
  • Assessing anterior and lateral hemithorax views bilaterally to evaluate for pneumothorax and pleural effusion

Core Skills Training

E-FAST for Trauma Examination

As a part of this E-FAST for Trauma examination procedure, the learner will begin by introducing themselves to the patient, explaining the purpose of the scan, and describing what the patient can expect during the exam. After positioning the ultrasound machine and following standard infection control practices, the learner will proceed to obtain five key views consisting of the right upper quadrant (RUQ), left upper quadrant (LUQ), two suprapubic pelvic views (transverse and sagittal), cardiac views (subxiphoid and parasternal long), and bilateral lung views.
The learner will analyze the acquired images and promptly communicate findings to the trauma team leader.
All necessary affordances are provided to complete the procedure with efficiency.

Training

With prompts, guidance and affordances learners are hand-held through the process to practice the procedure in a virtual environment with a virtual patient.

  • Photorealistic virtual environment
  • Physics-based interactions
  • Detailed instructions
  • Adequate affordances to assist in task completion

Assessment

Test acquired skills to perform the procedures from start to finish without prompts. An incorrect step will take the learner back to the start to start afresh.

  • Live scoring
  • Instant feedback
  • Adequate affordances for efficient performance
  • Time tracking to monitor activity completion
